Requirements
- Current Registered Nurse (RN) license in the State of Maine.
- Minimum of one year of recent clinical nursing experience preferred
- Current CPR certification.
- Reliable transportation with valid driver's license and current automobile insurance.
Nice To Haves
- Experience in hospice, home health, palliative care, or community health is preferred.
- ANA certification in Home Health or Community Health Nursing is encouraged.
- Excellent assessment, communication, and critical thinking skills.
Responsibilities
- Deliver skilled nursing care to patients in their homes/facilities in accordance with physician orders and the individualized plan of care.
- Complete comprehensive assessments to identify each patient's physical, emotional, psychosocial, and spiritual needs.
- Develop, implement, and update patient-centered care plans in collaboration with patients, families, caregivers, and the interdisciplinary team.
- Educate patients and families about disease progression, symptom management, medications, and available resources to support informed decision-making.
- Provide expert pain and symptom management to promote comfort and quality of life.
- Coordinate care across disciplines to ensure seamless communication and continuity of services.
- Maintain accurate, timely documentation in accordance with agency, state, and federal guidelines.
- Advocate for patients and families while supporting their goals and wishes throughout the hospice journey.
