About The Position

ChristianaCare – Wilmington Hospital is recruiting an RN Case Manager for its Weekend Incentive Program (WIP). As a member of the interdisciplinary team, the RN Case Manager creates, implements, and monitors treatment plans to support safe, timely, and effective transitions across the care continuum and discharge planning. The Care Management Triad Team Model brings together the RN Case Manager, who manages patient care and discharge planning; a Social Worker, who addresses psychosocial barriers and discharge needs; and Utilization Management, which reviews patient status for appropriateness and anticipated payer coverage. The schedule requires working three out of four weekends in a four-week schedule: 12-hour day shifts on Saturday and Sunday, plus one rotating Friday evening shift. An annual rotating holiday is required every other year. The WIP includes a special rate of pay and full benefits, except paid time off. The position is flat-rate and merit eligible. With benefits, the stated rate is $35 plus a $10 shift differential; without benefits, the rate is $40 plus a $20 shift differential. A sign-on bonus is available to external candidates only.
