About The Position

Heartline Staffing is recruiting an experienced Registered Nurse Case Manager for a full-time Senior Care Options (SCO) opportunity in Lynn, Massachusetts. This position offers an exceptional work-life balance with a hybrid schedule of three days in the community and two days remote, Monday–Friday daytime hours, and no nights, weekends, or holidays.

The RN Case Manager provides comprehensive case management for assigned SCO members with complex needs, coordinating cost-effective, quality care through individualized assessment, care planning, implementation, and ongoing evaluation.

This is a community-focused case management position requiring frequent local travel. The RN works with members and families in their home environment while coordinating services and care across the healthcare continuum.

Requirements

• Active Massachusetts Registered Nurse License REQUIRED
• Minimum 3 Years of Licensed Registered Nurse Experience REQUIRED
• Home Care Clinical Experience REQUIRED
• Ability to Manage a Caseload of Members with Complex Needs REQUIRED
• Care Planning, Implementation & Evaluation Experience REQUIRED
• Ability to Travel Locally on a Frequent Basis REQUIRED
• Experience with Case Management Data Systems REQUIRED
• Proficiency with Microsoft Office Tools REQUIRED

Nice To Haves

• Bachelor of Science in Nursing PREFERRED

Responsibilities

• Complete comprehensive assessments of member needs, condition, and environment
• Develop individualized plans of care corresponding with physician orders
• Regularly evaluate and update patient care plans as needs change
• Maintain ongoing responsibility for an assigned caseload
• Oversee implementation and evaluation of individual plans of care
• Ensure delivery of competent, quality patient care in the home
• Coordinate care with physicians and interdisciplinary health team members
• Maintain communication with Commonwealth Care Alliance (CCA)
• Collaborate with the provider community as appropriate
• Work directly with members and their families
• Provide patient education
• Maintain required case-management documentation
• Participate in team knowledge-sharing activities
