About The Position
Incumbent serves as an Outpatient/Inpatient Nurse Case Manager for specified high-risk patients, focusing on specialty clinics needing specific Case Management. The role involves facilitating care transitions, ensuring quality and cost-effective healthcare, and functioning as a resource on transitional and regulatory issues.
Requirements
- Associate's or Bachelor's Degree in Nursing.
- Valid, unrestricted Professional Nursing License to practice in any U.S. state, Puerto Rico, or a U.S. territory.
- Current Basic Life Support (BLS) certification by the American Heart Association.
- Three years of clinical nursing experience, with one year in an Outpatient/Inpatient setting.
- Knowledge of case management, contract health, and utilization review processes.
Responsibilities
- Assess care plans and monitor progress with an interdisciplinary team.
- Evaluate Emergency Department (ED) patients for medical necessity and care coordination.
- Propose and implement outpatient plans for safe discharge from the ED.
- Coordinate care for patients with chronic pain, abuse issues, and frequent utilization problems.
- Interact with patients across all age groups to help them access necessary resources.
