About The Position

This role involves providing case management services as a Registered Nurse (RN). The position requires a strong understanding of the hospice philosophy, principles of death and dying, pain and symptom management, and adult teaching/learning principles. The RN Case Manager will be responsible for providing basic and skilled nursing care, setting priorities, and assessing intervention limits. They must be able to work independently and as part of an interdisciplinary team, demonstrating strong organizational, time management, and prioritization skills.

Requirements

  • Graduate of an accredited school of registered nursing.
  • Currently licensed as an RN through the Ohio Board of Nursing.
  • Must be able to obtain a license in the state of Michigan.
  • Working experience charting in an EMR system.
  • Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills.
  • Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention.
  • Possesses strong verbal and written communication skills.
  • Demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
  • Has the ability to work independently and within an interdisciplinary team.
  • Demonstrates strong organizational, time management, and prioritization skills.
  • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
  • Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.
  • Meets state requirements for maintaining professional license.
  • Must be CPR certified according to the American Heart Association CPR standards.

Nice To Haves

  • BSN preferred.
  • Minimum 2 years of medical-surgical nursing experience preferred.
  • Previous experience in homecare, and/or hospice experience is preferred.
  • Netsmart system experience preferred.
