VNA is looking for a RN Case Manager. The RN Case Manager works collaboratively with physicians and the interdisciplinary care team to assess, plan, coordinate, and deliver skilled home health care to patients and their families. This role focuses on providing supportive, patient-centered care that promotes comfort, safety, and quality of life. The nurse utilizes the nursing process to manage patient care, coordinates services with internal and external providers, and ensures compliance with payer guidelines. The RN Case Manager also prepares patients and caregivers for discharge when appropriate and connects them with community resources to support continuity of care.

Requirements

  • Active Registered Nurse license in the California
  • Minimum of one year nursing experience required
  • Home Health experience required
  • Current driver's license
  • Auto insurance
  • Current CPR certification
  • Reliable vehicle

Nice To Haves

  • Two years of recent medical-surgical nursing experience preferred
  • This position is based in Monterey, CA and requires daily travel throughout the Monterey Peninsula and surrounding coastal communities. Candidates must reside within a reasonable commuting distance that allows efficient coverage of the assigned territory.
