About The Position

Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, founded in 1982. They offer exceptional serious illness care, hospice care, and grief support for adults and children. Most direct patient care is provided in the field, with team members caring for patients in their homes, nursing facilities, or assisted living facilities. They also operate a 24-bed inpatient unit in Joliet, and non-clinical staff are based at their Joliet administrative offices. The Field Registered Nurse (RN) Case Manager, as part of the interdisciplinary team, is responsible for skilled nursing assessment and hospice care to maximize patient and family/caregiver comfort and health, in line with Lightways policies. The RN plans and directs services to meet the needs of individuals and families/caregivers in their homes and communities.

Requirements

Current RN license
Graduate of accredited nursing program
Minimum of 2 years of nursing experience
Valid driver’s license and insured, reliable vehicle

Nice To Haves

At least 1 year in public health, home care, or hospice preferred

Responsibilities

Deliver skilled nursing care and hospice services in home and community settings
Conduct comprehensive patient assessments and develop individualized care plans
Administer medications and treatments while monitoring patient symptoms and outcomes
Educate patients and caregivers on care, self-management, and disease progression
Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
Maintain accurate documentation and participate in on-call coverage as needed
