The RN Case Manager is responsible for initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs. This includes assessing for risks such as grief, cultural and spiritual needs, and verbal and non-verbal communication. The role involves implementing and revising the individualized plan of care, collaborating with patients, caregivers, physicians, and healthcare professionals, and participating in Inter-Disciplinary Group (IDG) meetings. The RN Case Manager will also perform on-call responsibilities, ensure timely documentation, and maintain compliance with all relevant laws and regulations.
