About The Position

Home Care Network, Inc. (HCN) is seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Recertification Nurse to join our agency and serve existing and returning clients throughout Northwest Ohio in a full-time capacity. This is a field staff leadership role where the RNCM has a passion for delivering consistently excellent care and care experiences to clients, and the ability to do so directly and through the thoughtful leadership of others. This position requires travel to multiple counties but offers a consistent schedule to fit personal/family commitments (no overnight shifts or overnight travel). Services primarily include recertifications, however opportunities for starts of care (SOC), resumptions of care (ROC), and other assessments and services are available (not required), as well as new hire orientations and supervisory reviews. For those pursuing career growth, a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported.

Requirements

  • At least 1 year of experience in Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments.
  • At least 1 year of paid experience at a home health agency (or equivalent) is required.
  • Willingness to travel across a 2-hour radius across NorthWest Ohio.
  • Weekday position (Monday - Friday) requiring both daytime and evening availability.
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
  • Familiarity with HIPAA regulations and medical terminology.
  • Willingness and ability to perform required physical demands.
  • Consent + Ability to pass a State & Federal background check (including fingerprints).
  • 18yo + active Driver's License / State ID + US Work Authorization.
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).

Nice To Haves

  • Nurse Supervision experience is a plus.
  • 2-5 years' experience in home health agency or equivalent is preferred.
  • 1 or more years of experience in wound care, lab draws, and OASIS assessments is a plus.

Responsibilities

  • Case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services.
  • Occasional new hire orientations, in-service and supervisory reviews.
  • Input to influence training and education, care practices and outcomes, and the success of clients, employees and business overall.
  • Back-up for occasional Skilled Nursing visits including med sets, dressing changes, etc.
  • Coordinating, re-evaluating and confirming the medical necessity of continued home health care services for patients.
  • Providing patient care and ensuring optimal outcomes.
  • Overseeing patient services and providing expert nursing care.
  • Submitting updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • Ensuring successful communication with and between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
  • Performing client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Managing/supervising a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Documenting/Communicating/Coordinating patient status/change in condition to care team.
  • Performing job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
