Elara Caring serves over 60,000 patients nationwide, and we are expanding our Private Duty Nursing team in the Fort Wayne area. We seek compassionate Registered Nurse Case Managers who desire autonomy, flexibility, and the opportunity to profoundly impact patients' lives. Private Duty Case Management allows nurses to concentrate on one patient and family, foster long-term relationships, exercise greater independence in clinical practice, coordinate care that genuinely enhances quality of life, and experience a less chaotic environment compared to hospital bedside care.

Requirements

  • Current Registered Nurse (RN) license in Indiana
  • Graduate of an accredited RN program
  • Strong communication and patient care coordination skills
  • Ability to work collaboratively within an interdisciplinary team
  • Ability to lift, push, pull, or support up to 50 pounds as needed

Nice To Haves

  • 2+ years clinical nursing experience
  • Home Health or Private Duty nursing experience
  • Case management experience

Responsibilities

  • Assess patient health needs and determine appropriateness for home care admission
  • Complete admission assessments and develop individualized plans of care
  • Communicate with physicians, caregivers, and referral sources to ensure smooth care transitions
  • Coordinate services including equipment, supplies, pharmacy, and additional medical support
  • Supervise Private Duty RNs, LPNs, and Home Health Aides to ensure quality and continuity of care
  • Review clinical documentation for accuracy and compliance with the plan of care
  • Evaluate patient progress and adjust care plans when necessary
  • Provide education and resources to patients and caregivers
