About The Position

The RN Case Manager Oncology provides individualized, goal-directed nursing care to patients through the use of the Nursing Process. The RN functions independently with little supervision. This role involves assessing patients, planning and implementing nursing care, and evaluating patient outcomes, with a focus on oncology patients. The position requires knowledge of human growth and development across the lifespan and the ability to provide age-specific care. Responsibilities include administering nursing care, demonstrating technical skills in procedures, medications, IV therapy, chemotherapy, radiation, and equipment management, as well as recognizing and functioning appropriately under stress and in crisis situations. Effective communication, anticipation of psycho-social and spiritual needs, and facilitation of patient and family teaching are also key components. The role also involves evaluating patient status, modifying the plan of care, and ensuring accurate and thorough documentation.

Requirements

Responsibilities

  • Assess all assigned patients prior to seeing the physician, including physical and mental health, reaction to illness, treatment requirements, current medication or chemotherapy regimen, vital signs, pain control, and toxicity assessments.
  • Initiate Nursing Care Plans that identify the patient’s present/potential problems from the patient assessment and formulate a nursing diagnosis or problem statement.
  • Collaborate with MD on the plan of care based on collected data, identified problems, and established goals with patient/family input.
  • Formulate nursing orders that delineate interventions to be taken relating specifically to identified patient/family needs including education, discharge planning, and follow-up care.
  • Administer nursing care according to MD’s orders and nursing plan of care within the scope of hospital and nursing department policies and procedures.
  • Demonstrate accurate technical skill and operational knowledge of: Procedures, Medications, IV therapy, Chemotherapy, Radiation, Equipment, Side effect management, Telephone triage when applicable to their current role.
  • Demonstrate ability to recognize and function appropriately under stress and in a crisis situation.
  • Communicate (verbally and written) the results of the nursing assessment, the plan of care, and the patient’s response to care to all individuals involved in patient care.
  • Anticipate psycho-social and spiritual needs of the patient involving appropriate services as needed.
  • Facilitate and implement patient and family teaching.
  • Recognize the needs for additional patient care services, and coordinate these activities with appropriate individuals or outside agency.
  • Evaluate patient’s status and interventions and modify plan as needed with input from care team members and patient/family.
  • Record the evaluation of the patient’s response to nursing intervention.
  • Evaluate patient and family preparedness for the care of the patient at home.
  • Ensure accuracy and relevancy of patient records and other hospital documents.
  • Document legibly on the patient record and be thorough, accurate, and complete.
  • Document errors in charting according to policy.
  • Review and maintain pain contracts according to hospital policy and procedure.
  • Use appropriate terminology and approved hospital abbreviations.
  • Review and sign off patient chart at end of clinic.
  • Maintain department specific skill competencies.
  • Satisfactorily complete annual competency review.
  • Consistently demonstrate skills necessary to provide competent care to all assigned patients.
  • Demonstrate initiative to acquire skills necessary to provide new care treatments or services.
