About The Position
The RN Case Manager cares for patients with terminal illnesses or other diagnoses, indicating they have fewer than six months to live. The RN Case Manager will provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life. The RN Case Manager is typically part of a care team. This team collaborates to ensure individuals, and their families have the physical, emotional, and spiritual support they require throughout the care process.
Requirements
- Cares for patients with terminal illnesses or other diagnoses, indicating they have fewer than six months to live.
Responsibilities
- Provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life.
- Collaborate with a care team to ensure individuals and their families have the physical, emotional, and spiritual support they require throughout the care process.
- Provide medical social services to the patient and/or family according to the Plan of Care.
- Perform all assignments with honesty, integrity, and care that promotes the mission, vision, and values of Friendship Village.
