About The Position
JourneyCare Hospice is seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Northbrook area. They offer a different approach with manageable caseloads, strong interdisciplinary support, and dedicated time for patient and family care, contrasting with high caseloads and frequent weekend rotations found elsewhere. This role focuses on quality of care over visit volume, with supportive leadership and team autonomy.
Requirements
- Graduate of an accredited RN program
- Active Illinois RN license
- Current CPR and First Aid certification
- Valid driver's license and proof of insurance
Nice To Haves
- Hospice or home health experience preferred
- Strong clinical judgment and communication skills
Responsibilities
- Manage a dedicated hospice caseload as the primary RN Case Manager
- Perform patient assessments, admissions, and individualized care planning
- Deliver skilled nursing care focused on comfort and dignity
- Coordinate care with physicians and interdisciplinary team members
- Conduct routine visits and adjust care plans as needed
- Provide end-of-life support, including family education and pronouncement
- Maintain timely, accurate documentation
- Supervise Home Health Aides in compliance with regulations
