Assisted is looking for a full-time or per diem RN Case Managers to join our Hospice Team in our Ventura branch. Spanish speaking is required. Requirements
- Graduate of an approved school of professional nursing.
- Current, active license in the state of California Board of Nursing
- Current CPR
- health clearance
- TB clearance
- Spanish speaking is required.
Responsibilities
- Complete initial and on-going comprehensive assessments of the patient’s needs
- Provide service and /or treatments requiring substantial and specialized nursing skills
- Initiate appropriate preventative and rehabilitative nursing procedures
- Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs
