About The Position

This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.

Requirements

Graduate of an accredited school of nursing; ASN or BSN
Currently Licensed as a Registered Nurse in the State of Michigan
Minimum one (1) year experience as a nurse in a clinical care setting
Reliable transportation, valid driver’s license, and valid automobile insurance coverage
Nursing skills as defined by accepted nursing standards
Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
Ability to handle multiple priorities, documentation requirements and deadlines

Nice To Haves

Hospice or Home Health experience preferred

Responsibilities

Coordinate comprehensive care for caseload.
Address the physical, emotional, and spiritual needs of patients.
Build rapport with patients, families, physicians, facilities, and other interdisciplinary team members.
Provide personalized, compassionate care to patients and families.
Daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.
