RN Case Manager (Hospice)
About The Position
The RN Case Manager at Lifted Hospice plays a vital role in providing compassionate, clinically excellent end-of-life care. This position combines clinical expertise with genuine empathy to deliver personalized nursing care in the patient's home or care setting. The RN Case Manager collaborates with an interdisciplinary team to develop and implement care plans that align with patient goals, support families, and meet high standards of care. A key aspect of the role involves educating and equipping families to help them understand the end-of-life process, provide comfort, and feel confident in their caregiving role.
Requirements
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program, with an active state license and clinical skills that meet current standards of nursing practice.
- Experience as a registered nurse in a clinical care setting.
- Must read, write and comprehend English.
- Current driver’s license, reliable transportation, and valid auto liability insurance.
Nice To Haves
- Hospice experience preferred.
- Excellent interpersonal skills.
Responsibilities
- Completing thorough assessments to guide care based on each patient’s physical, emotional, and clinical needs.
- Delivering skilled hospice care to help manage pain and improve quality of life.
- Collaborating with the full care team, the patient, and their loved ones to create and follow a unified care plan.
- Documenting care and updates accurately in line with all standards and requirements.
- Educating and encouraging caregivers, offering emotional support, and helping families feel prepared and confident.
