About The Position

This role is within Sanford Hospice, where the focus is on supporting patients and their loved ones with compassion and understanding. The RN Case Manager provides care to assigned patients in a caring, safe, and efficient manner, demonstrating leadership to staff and adhering to location standards, procedures, and individualized patient plans of care. The position involves evaluating outcomes, understanding the unit of care (family and patient), and possessing knowledge of hospice palliative care, hospice philosophy, and scope of services. The role requires recognizing cultural attitudes towards death and dying, identifying the five stages of grieving, and explaining advanced directives, clinical requirements, and interdisciplinary services. The nurse utilizes independent judgment in healthcare integration, participates in the creation and implementation of the plan of care with the interdisciplinary group, and applies age-specific care methodologies for neonatal, pediatric, adolescent, adult, and geriatric patients. Responsibilities include interviewing potential residents, assessing residents in compliance with regulations, establishing and evaluating service plans, and communicating effectively with patients, families, and the healthcare team. The role demands independence, assertiveness, critical thinking, clear documentation, problem-solving skills, self-motivation, and the ability to work with minimal direction. It also requires incorporating teaching/learning principles, adapting teaching to age groups, facilitating groups, implementing projects, establishing priorities, and demonstrating flexibility and organizational skills. Knowledge of reimbursement for services is essential. The position may involve exposure to communicable diseases, hazardous materials, and potential injury. The role is subject to multiple sensory and environmental stressors.

Requirements

  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencu m b ered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Must possess a valid driver’s license.

  • Interviews potential residents and assesses residents in compliance with regulations, policy and procedure.
  • Uses assessment information for admission and continued stay in the hospice community.
  • Establishes a service plan for each resident and evaluates on an on-going basis.
  • Communicates with patients/residents, family members and others on the health care team.
  • Demonstrates independence, assertiveness and critical thinking when working with patients and co-workers.
