Requirements

  • 1 year of clinical nursing experience
  • Associate's or Bachelor's degree
  • Graduation from an approved school of nursing
  • RN (Registered Nurse) Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state
  • BLS (Basic Life Support) Issued by American Red Cross or American Heart Association within 30 days of hire
  • Must be able to communicate effectively in English (verbal/written).
  • This job requires the incumbents to operate a INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Oklahoma State Drivers License as well as a driving record which is acceptable to our insurance carrier.

Responsibilities

  • Manages/coordinates all aspects of service delivery as defined by physician orders
  • Participates in interdisciplinary group meetings/re-certification process.
  • The Case Manager is directly responsible for all aspects of the patient's care.
  • The plan of care is discussed with all the disciplines and reevaluated as needed.
  • Ensures implementation of activities for improved quality of patient care
  • Supervises Home Health Aide activities and plan of care
  • Supervises and educates nursing/medical students during their rotation
  • Specializes in pain and symptom management issues in patient care
