About The Position

BrightStar Care is seeking an RN Case Manager for their South Minneapolis Area location, working 4 days per week. This role involves working with a team of nurses to complete assessments, re-assessments, and some skilled cares for home care private pay and veteran clients. As a non-Medicare agency, OASIS documentation is not required. The company emphasizes a team and family atmosphere focused on keeping clients safe, happy, and healthy in their homes. PTO accrues from Day 1, and mileage between clients and the office is paid. The role primarily serves clients in the Minneapolis Metro Area (Bloomington, Edina, Eden Prairie, etc.). Requirements

Active RN license in the state of Minnesota.
Previous experience in a home health or acute care setting is advantageous.
Exceptional interpersonal and communication skills.

Responsibilities

Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
Collaborate with multidisciplinary team members to optimize patient care and outcomes.
Educate patients and their families on health maintenance and disease prevention.
Ensure accurate documentation and compliance with all regulatory guidelines.
