The RN Case Manager integrates health care for clients from pre-admission to post discharge through the coordination and sequencing of care. This integration enhances patient/resident flow and interdisciplinary communication, promoting early intervention. The position seeks to evaluate outcomes based on integration among clinical, financial, and utilization data. The role functions in structured and unstructured healthcare settings, utilizing independent nursing judgment. Knowledge of and utilization of appropriate age-specific structured care methodologies, such as protocols/integrated clinical pathways/guidelines/standards of care, relating to the overall healthcare needs of neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents is required. The ability to work with growth and development needs of client populations in the clinical area, communicate with patients/residents, family members, and the healthcare team, and coordinate, facilitate, and negotiate with others is essential. The role involves monitoring, evaluating, and trending patient/resident responses using structured care methodologies, while being fiscally responsible and controlling variances and duplication. Independence, assertiveness, and critical thinking are crucial when working with patients/residents and co-workers. The ability to document clearly, solve problems, and be self-motivated with little direction is necessary. Incorporating teaching/learning principles and adapting teaching to age groups, facilitating groups, and implementing projects are also key. Establishing priorities, strong flexibility, and organizational skills are required. Knowledge about reimbursement for services provided is essential.

Requirements

Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
Currently holds an unenciphere RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
Obtains and subsequently maintains required department specific competencies and certifications.
Ability to communicate with patients/residents, family members and others on the health care team.
Ability to coordinate, facilitate, and negotiate with others.
Ability to monitor, evaluate and trend patient/resident responses utilizing structured care methodologies.
Ability to demonstrate independence, assertiveness and critical thinking when working with patients/residents and co-workers.
Ability to document clearly.
Ability to solve problems.
Self motivated.

Nice To Haves

Bachelor's degree in Nursing strongly preferred.
Minimum of two years’ clinical experience preferred.
Depending on location, a valid drivers license may be required.
