Requirements

Active Florida RN license in good standing
Minimum 2 years of clinical nursing experience, preferably in home health or community care
Valid Basic Life Support (BLS) certification
Strong assessment and clinical documentation skills
Reliable transportation and valid driver's license and valid insurance for in-home visits throughout Miami-Dade County
Excellent communication and interpersonal skills with patients and families
Ability to work independently and manage time effectively in a field-based role
Excellent verbal and written communication skills in English

Nice To Haves

Experience with OASIS assessments or home health intake evaluations is a plus
bilingual (English/Spanish) a plus

Responsibilities

Perform skilled nursing evaluations and document clinical findings accurately
Assist with competency assessments of caregivers and conduct supervisory visits
Develop and communicate individualized care plans based on assessment outcomes
Collaborate with care coordinators and home health aides to ensure continuity of care
Ensure assessments comply with state, federal, and agency regulatory standards
Communicate non-medical evaluation results and recommendations clearly to patients, families, and care teams
Maintain accurate and timely clinical documentation in compliance with home health guidelines
