Position Overview: Tier I - The RN Case Manager I, Emergency Department/Critical Care Unit (ED/CCU) works with patients and families admitted into the Emergency Department and/or Critical Care Unit along with the healthcare team in the collaborative development of a plan for discharge and arrangement of any post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care. The RN Case Manager I (ED/CCU) is a temporary entry-level position, and requires advancement to the RN Case Manager II level within one (1) year from the start date. Tier II - The RN Case Manager II, Emergency Department/Critical Care Unit (ED/CCU) works with patients and families admitted into the Emergency Department and/or Critical Care Unit along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care. Tier III - The RN Case Manager III, Emergency Department/Critical Care Unit (ED/CCU) works with patients and families admitted into the Emergency Department and/or Critical Care Unit along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services.
