About The Position

Position Overview: Tier I - The RN Case Manager I, Emergency Department/Critical Care Unit (ED/CCU) works with patients and families admitted into the Emergency Department and/or Critical Care Unit along with the healthcare team in the collaborative development of a plan for discharge and arrangement of any post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care. The RN Case Manager I (ED/CCU) is a temporary entry-level position, and requires advancement to the RN Case Manager II level within one (1) year from the start date. Tier II - The RN Case Manager II, Emergency Department/Critical Care Unit (ED/CCU) works with patients and families admitted into the Emergency Department and/or Critical Care Unit along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care. Tier III - The RN Case Manager III, Emergency Department/Critical Care Unit (ED/CCU) works with patients and families admitted into the Emergency Department and/or Critical Care Unit along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care.

Requirements

  • Associate Degree in Nursing (ADN)
  • Current CA Registered Nurse (RN) license
  • Current Basic Life Support (BLS) certification

Nice To Haves

  • Bachelors of Science in Nursing (BSN)
  • Bilingual in Spanish
  • One (1) year of experience in Case Management and strong discharge planning skills
  • One (1) year of experience with InterQual
  • One (1) year of experience with Allscripts and Meditech
