RN Case Manager - Acute Care - Day Shift
About The Position
ChristianaCare Union Hospital is seeking an RN Case Manager with experience in Utilization Management and Acute Care Case Management. The role involves the utilization of healthcare resources, including discharge planning, to achieve clinical, quality, financial, and patient satisfaction goals. This position requires ongoing contact with patients/families, medical-dental staff, nursing, social work, PI-outcome coordinators, disease management, and ancillary services. Additional contact includes home health care providers, durable medical equipment providers, insurance/managed care organizations, and community resources.
Requirements
- Maryland or Compact State RN Licensure Required
- Current BLS
- Case Management Certification (CCM) required within 18 months of eligibility
- Strong interpersonal skills relative to both professional and lay interactions
- Strong organizational skills
- Working knowledge of Performance Improvement activities
- Working knowledge of data management/reporting practices
Nice To Haves
- Relevant work experience in Utilization Management, Case Management, or Home Care is preferred.
