About The Position

St. Mark's Hospital is seeking a PRN Registered Nurse Case Manager to manage rehabilitative care and support optimum patient outcomes. The Case Manager provides, coordinates, and directs care according to each rehabilitation patient's needs, collaborates with the Rehab Program Director and Facility Case Management Director on departmental functions, and coordinates efforts across the Rehabilitation team. The ideal candidate is available for at least two shifts per week and has some weekend availability. The Case Manager orients patients, families, and caregivers to the program, including the case manager's role, rehabilitation philosophy, continued-stay and discharge criteria, Medicare and insurance benefits, grievance procedures, the treatment-plan process, and patient rights and responsibilities. The role coordinates patient and caregiver education and encourages their participation in team discussions about plans, goals, and status, including through family conferences. The Case Manager supports implementation of treatment plans aligned with patients' strengths, abilities, needs, and preferences; facilitates patient involvement throughout rehabilitation; documents discharge-planning evaluations and psychosocial assessments; and communicates social, financial, and discharge needs and preferences. The role is also accountable for promoting consistent, positive patient interactions that advance patient service. St. Mark's Hospital is a 308-plus-bed hospital serving Salt Lake City and surrounding communities. It provides a broad range of clinical services and is part of HCA Healthcare. Requirements

Registered Nurse.
Associate degree in nursing.
Basic Cardiac Life Support.
At least 2 years of experience in hospital case management or at least 3 years of experience in critical care nursing.
Ideal candidate availability: at least 2 shifts per week with some weekend availability.

Nice To Haves

Bachelor of Science in Nursing (BSN).
InterQual experience.
Certification in Case Management.

Responsibilities

Provide program orientation to patients, families, and caregivers, covering the case manager's role, rehabilitation philosophy, continued-stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment-plan processes, and rights and responsibilities.
Coordinate patient, family, and caregiver education.
Promote patient, family, and caregiver participation in team discussions about plans, goals, and status, including through family conferences and other interactions.
Ensure implementation of treatment plans that support each patient's strengths, abilities, needs, and preferences.
Facilitate patient involvement throughout the rehabilitation process.
Document findings from discharge-planning evaluations and psychosocial assessments.
Communicate patients' and families' social, financial, and discharge needs and preferences.
Promote consistent, positive patient interactions that advance patient service.
Collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions.
Coordinate efforts within the Rehabilitation team.
