About The Position

Hartford HealthCare is Connecticut’s most comprehensive healthcare network, with Hartford Hospital being one of New England's largest and most respected teaching hospitals and a Level 1 Trauma Center. The hospital is known for its cutting-edge treatment, including the largest robotic surgery center in the Northeast and the Center for Education, Simulation and Innovation (CESI). The Case Coordination Department at Hartford Hospital is responsible for assessing patient needs for appropriate care levels across the continuum. The staff consists of Registered Nurses experienced in discharge planning and case management, supported by administrative assistants to ensure smooth patient transitions. The RN Case Coordinator position at Hartford Hospital focuses on facilitating patient throughput for a select inpatient population. This role involves assessing patients for appropriate care levels, coordinating care and services across the continuum, identifying risk factors, facilitating support services, performing utilization management, developing and implementing transition planning, documenting care in case management applications, and collaborating on process and quality improvement initiatives. The role offers an opportunity to be a key member of a collaborative nursing team, prioritizing patient care.

Requirements

  • Possess a CT RN license
  • ASN required
  • Three years acute-care, and diversified clinical nursing experience, including medical/surgical nursing
  • Excellent communication, negotiation and conflict resolution skills

Nice To Haves

  • BSN preferred
  • Case management, home care, and/or managed care experience strongly considered
  • Knowledge of computer applications and reimbursement systems preferred
  • Certification in case management desired
