About The Position

POSITION SUMMARY: The Geriatrics Case Coordinator coordinates the case processing functions within Elders Living at Home Program. The Case Coordinators are responsible for providing intensive assessment and case management services to homeless and at-risk elders referred to the program, with the goal of helping them transition to and then maintain safe, affordable permanent housing. Position: Case Coordinator Department: ELAHP Schedule: Full Time

Requirements

EDUCATION: Bachelors' degree in psychology, social work, geriatric management, public health or related field (or equivalent combination of formal education and related experience may be substituted for degree).
EXPERIENCE: Prior experience working with adolescents, teens and adults in the community required.
KNOWLEDGE AND SKILLS: Excellent communication, interpersonal, organizational, and time management skills.
Cultural sensitivity and comfort with a wide range of social, racial and ethnic populations.
Computer competency, inclusive of the ability to enter and access data, required.

Nice To Haves

Knowledge of health, wellness/sport and recreation a plus.
Additional language skills (beyond that of English) appropriate to the patient population served is very helpful.
Individuals from diverse cultural backgrounds preferred.
Excellent interpersonal skills to instill confidence and to advocate for patients and their family.
Motivation and self-direction; ability to prioritize competing responsibilities in fast-paced clinical care setting.
