RN Care Transition Coordinator
About The Position
The RN Care Transition Coordinator position is responsible for providing care transition services for Providence hospitalized patients. The Coordinator works with the Providence physician team and hospital discharge planning staff to ensure patients are successfully discharged from the hospital with no disruption in services. The position encompasses the roles of nursing assessment, liaison abilities, communication, education and team/family support. Providence caregivers are not simply valued – they’re invaluable. Requirements
- Graduate from an accredited school of Nursing.
- Washington Registered Nurse License upon hire.
Nice To Haves
- Bachelor's Degree in Nursing.
- 1 year Clinical experience.
- Experience in providing patient and staff education.
- Experience in conflict resolution.
