Collaborate with Samaritan team members to manage the hospital palliative consult census remotely. Serve as a remote liaison for hospitalized Samaritan patients of all programs. Conduct virtual/phone consultations with patients and families to assess needs, provide education, and support eligibility review. Communicate outcomes and plans to hospital teams and Samaritan providers. This includes families, facilities and discharge planner/case management. Work with other facility liaisons in hospital settings when discharging to their facility. Collaborate with hospital referral sources to coordinate education, follow-up, and discharge planning. Ensure seamless transitions from inpatient to outpatient Samaritan programs. Communication to program team members including providers as needed. Gather necessary documents such as consents for programs, medical records, and insurance information. Organize and support discharge planning remotely, including DME, medication needs, and outpatient scheduling. Document in the EMR in a timely manner. Complete patient logs daily for initial and follow up encounters. Attend virtual hospital or interdisciplinary meetings if requested. Provide education and support to facility staff, families, regarding goals of care, advanced care planning and having difficult conversations e.g. hospice transition when appropriate. Possible weekend remote assignment on a rotation. Assist hospital team with iGIP hospital coverage when needed. Perform other duties as assigned.
