About The Position
Cheer Home Care is seeking an RN Care Navigator to make a positive impact in the community by providing high-quality care to older adults in their homes. The role emphasizes connection, compassion, and making a difference beyond just tasks. The RN Care Navigator will collaborate with interdisciplinary team members to develop and implement individualized care plans, providing constant feedback to clients, client representatives, caregivers, and nurses. This position requires working closely with the office team, demonstrating excellent communication and problem-solving skills. The primary responsibility is to serve as the care manager, coordinating care among caregivers, RNs, therapists, and outside representatives.
Requirements
- Upbeat Personality
- Active California RN License
- Strong Computer Skills
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance
- Valid driver’s license and proof of insurance
- Cell phone with Internet Access
- Proof of Negative TB results
- Willing to Work One Weekend a Month as Back-Up On Call
