About The Position
Cheer Home Care is looking for an RN Care Navigator who loves spending time with older adults and provides high quality care in home. The RN Care Navigator collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, caregivers and nurses. They work closely with the office team and have excellent communication and problem-solving skills.
Requirements
- Active California RN License
- Strong Computer Skills
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance
- Valid driver’s license and proof of insurance
- Cell phone with Internet Access
- Proof of Negative TB results
- Willing to Work One Weekend a Month as Back-Up On Call

Responsibilities
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Provide constant feedback to clients, client representatives, caregivers and nurses
- Work closely with the office team
- Serve as the primary care manager
- Coordinate care to the clients among caregivers, RNs, therapists and outside representatives
