RN Care Manager, Full-Time
About The Position
The Primary Care Registered Nurse (RN) Care Manager is responsible for the ongoing development, implementation, and evaluation of processes for the CWHS Case Management Plan, the ACO, and Commercial Payors. This role involves a highly collaborative process to assess, plan, implement, coordinate, monitor, and evaluate the services needed to meet the client's/patient's health and human service needs. The RN Care Manager acts as an advocate, communicator, collaborator, resource manager, and educator for interdisciplinary team members. Key responsibilities include assisting with Annual Wellness Visits, managing transitions of care post-ED and hospital discharge, and assessing patient needs across medical, functional, and psychological domains. The position requires broad knowledge of nursing practice, strong multitasking abilities, and excellent time management skills to ensure timely completion of all tasks.

Responsibilities
