About The Position
Community Care is hiring a RN Care Manager to join our PACE Program team. This is a full-time opportunity at our Racine location.
Requirements
- Associates degree in Nursing Required
- Wisconsin Licensed Registered Nurse
- Eligibility for long-term care functional screener certification is required
- WI Drivers License
- 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity
Nice To Haves
- Bachelors Degree preferred
- 2 years of work in community nursing
Responsibilities
- Completes discipline-specific initial and ongoing assessments.
- Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
- Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports.
- Follow up on all services to ensure quality/coordination and effectiveness of services.
- Complete all required documentation and charting and assists in the development and maintenance of member records.
