About The Position

Community Care is offering a $8,000 Sign-on Bonus for a Full-Time RN Care/Case Manager to join the Day Shift. This is a full-time opportunity for Outagamie County. The RN Care/Case Manager will collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs. They will conduct phone and in-person assessments using decision tools and resources to identify immediate and potential health needs, and complete all required documentation and charting and assist in the development and maintenance of member records.

Requirements

• Associate degree in nursing required.
• Minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required
• A current Wisconsin Registered Nurse license in good standing

Nice To Haves

• 2 years of recent case management preferred.
