The RN Care Manager role is a full-time position within the Emergency Department at Advocate Trinity Hospital. This role focuses on identifying targeted patients for intervention, conducting assessments, developing care plans, and initiating interventions within desired timeframes. The Care Manager collaborates with patients, families, and the ED team to achieve patient and organizational goals, focusing on care needs, choices, and satisfaction. They provide continuity of care and discharge planning services compliant with regulatory standards, ensuring patients and families are informed and supported. This includes managing various levels of health care, finances, housing, family discord, or illness adjustment, and utilizing community resources effectively. The role also involves initiating referrals, documenting interventions, performing reviews, and collaborating with various stakeholders to ensure efficient site of care determination and compliance with guidelines. The RN Care Manager must stay knowledgeable about healthcare regulations, reimbursement issues, and community resources, and complete UM screenings to recommend appropriate patient status. They develop and maintain relationships with community agencies and collaborate with Advocate Aurora Ambulatory Care Management and Continuing Health. Additionally, they serve as an educator and expert resource to ED and hospital staff on admission status, acute care criteria, utilization management, care coordination, discharge planning, and regulatory requirements. The position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan.
