About The Position

Vancouver Clinic is hiring a full-time RN Care Manager (36 hours/week) to support our Transitional Care Clinic. In this role, the RN Care Manager will coordinate patient care, helping ensure a seamless, supportive experience for patients and their families. This position is primarily focused on care coordination, patient and family education, psychosocial support, and improving quality of life through compassionate, collaborative care, with additional responsibilities that may include triage, administering treatments, and assisting with minor procedures. The RN Care Manager will embody Vancouver Clinic’s core values of compassion, excellence, integrity, teamwork, safety, and respect while maintaining required competencies for the role.

Requirements

  • Active, unencipere Washington or Multi-State RN license required.
  • Minimum 2 years clinical nursing experience required.
  • Ability to work, with or without reasonable accommodation, with a diverse population of patients and colleagues seeking or considering care in all areas is an essential function of all positions at the Clinic.

Nice To Haves

  • BSN or MSN/MN degree in nursing (BSN or MSN/MN) strongly preferred.
  • Relevant experience in one or more of the following healthcare areas preferred: Coordination of community resources, Care management of diverse patient populations, Ambulatory Care.
  • Case management certification strongly preferred.
