About The Position
We are seeking a compassionate and experienced Registered Nurse (RN) Care Manager to join our team. In this role, you will coordinate and deliver high-quality care for older adults, working closely with an interdisciplinary care team to support participants’ health, independence, and overall well-being.
Requirements
- Active Registered Nurse (RN) license (NC)
- Graduate of an accredited nursing program (BSN preferred)
- Minimum 2 years RN experience
- Experience working with elderly or frail populations required
- Home care experience preferred
- CPR certification (or willingness to obtain)
- Valid driver’s license and reliable transportation
- Strong clinical judgment and critical thinking skills
- Excellent communication and teamwork abilities
- Ability to work independently and manage multiple priorities
- Passion for providing person-centered care
Responsibilities
- Coordinate care with physicians, social workers, and interdisciplinary team members
- Perform assessments, triage, and ongoing monitoring of participants
- Develop and manage individualized care plans
- Provide skilled nursing care in clinic and home settings
- Educate participants, caregivers, and families
- Oversee medication management and chronic condition monitoring
- Ensure accurate documentation and regulatory compliance
- Support quality improvement and care coordination initiatives
