About The Position

Tryfacta is seeking a RN--Care Manager (Home Health) for our client in Sonora, CA 95370. This is a 13-week contract assignment. The work schedule is 07:00 AM-07:00 PM with 36 hours per week. The pay rate is $72-78/hr.

Requirements

Active Registered Nurse (RN) license in the state of practice.
Current BLS/CPR certification from an approved provider.
Strong patient assessment, care planning, and clinical decision-making skills.
Experience coordinating care with interdisciplinary healthcare teams.
Excellent patient and family education, communication, and teaching skills.
Valid Driver's License and be at least 21 years of age.
A reliable vehicle with current automobile insurance.
Ability to independently manage a home health patient caseload and maintain accurate documentation.

Nice To Haves

Home Health and/or Acute Care nursing experience.
Bachelor of Science in Nursing (BSN)
strong organizational and time management skills.

Responsibilities

Conduct comprehensive initial and ongoing assessments of patients in their homes.
Develop, implement, and update individualized nursing care plans based on patient needs.
Provide safe, skilled nursing care and monitor patients' health status.
Coordinate patient care with physicians, therapists, case managers, and other healthcare professionals.
Educate patients and their families on disease management, medications, treatments, and home care.
Accurately document patient assessments, interventions, progress, and changes in condition in a timely manner.
Identify and report abnormal findings or changes in patient condition and initiate appropriate follow-up care.
Update nursing plans, medication profiles, and treatment plans whenever patient needs change.
Perform preventive, rehabilitative, and therapeutic nursing procedures to support recovery and independence.
Comply with all organizational policies, legal regulations, documentation standards, and quality care requirements while managing an assigned home health caseload effectively.
