About The Position

Blue Cross & Blue Shield of Mississippi is seeking an RN Care Management Coordinator to serve as a liaison between patients and healthcare providers. The coordinator reviews patient needs, develops personalized care plans, and provides coordination services to improve outcomes. Working with providers and members, the clinician supports high-quality, patient-centered care and makes informed decisions about appropriate levels of care. The role requires clinical judgment, sensitivity, approachability, effective relationship-building, and patient-centered communication and advocacy.

Requirements

• Bachelor’s degree in nursing.
• Unrestricted RN licensure in the state of Mississippi.
• Intermediate proficiency with Microsoft Office Suite software, including Excel, Word, and PowerPoint.
• Strong oral and written communication skills.
• Clinical judgment, sensitivity to others’ needs, and ability to establish constructive and effective relationships.

Nice To Haves

• One year of clinical experience.
• Background in Hospital Case Management.

Responsibilities

• Perform clinical assessments to develop a Plan of Care, oversee discharge planning, provide education, and coordinate care.
• Identify members who require referral to Clinical Pharmacy, Quality Management, or Wellness Coaching.
• Collaborate with the clinical team on complex member cases to provide consistent care.
• Serve as a liaison between patients, families, and medical staff, listening to and considering their concerns.
