About The Position

Partnership Health Center (PHC) is an outpatient healthcare facility for employees and dependents covered under employer health plans. PHC offers a wide range of services, including primary and preventive care, behavioral health, physical therapy, pharmacy, and more. The RN Care Coordinator plays a vital role in facilitating care for high-risk or chronically ill patients. Working in collaboration with patients, caregivers, providers, and community partners, the Care Coordinator ensures effective care planning, communication, and follow-up across the healthcare continuum. This is a rewarding and dynamic position for an experienced RN who thrives in a collaborative environment and is passionate about improving patient outcomes. This position offers very flexible hours with no nights, weekends, or holiday shifts, while still providing full-time hours.

Requirements

  • Registered Nurse (RN), licensed in the State of New Jersey (required)
  • Graduate of an accredited nursing program
  • Clinical experience in one or more of the following settings (but not limited to): Hospital, Ambulatory or primary care, Post-acute care, Health plan or benefits management, Disease management or care coordination
  • Highly organized with strong verbal and written communication skills
  • Demonstrated ability to work independently and as part of a team
  • Strong decision-making, critical thinking, and problem-solving skills
  • Comfortable engaging with diverse patient populations and interdisciplinary teams
  • Ability to work with programs such as Microsoft Office (Word, Excel, PowerPoint, Access) and EHR systems
  • Maintains confidentiality in accordance with HIPAA and other regulatory guidelines
  • Professional, reliable, and compassionate

Nice To Haves

  • Educational background in Nursing, Public Health, Behavioral Science, etc. (preferred)

Responsibilities

  • Coordinate care for 10–15 patients per day through a mix of: Face-to-face visits, Telephonic outreach, Collaboration with treating providers, Documentation review, analysis, and care planning
  • Conduct extended patient engagements (30–45 minutes each) for approximately half the workday
  • Provide ongoing follow-up via secure email, phone calls, text messages, and other communications with: Patients and caregivers, Primary and specialty care providers, Community organizations and social services
  • Serve as a liaison between patients and the healthcare system to support care transitions and reduce barriers to care
  • Document all encounters, follow-ups, and care plans accurately in the electronic health record (EHR)
  • Assist with clinical duties as needed (e.g., vitals, vaccine administration, PPD readings)
