About The Position

The Hemophilia & Thrombosis Center (HTC) is a federally funded program dedicated to identifying, treating, and providing access to care for individuals with bleeding and clotting disorders across Colorado, Wyoming, and Montana. The center offers comprehensive diagnostic and treatment services, education, and consultative support for community physicians, as well as educational and support programs for patients and their families. The RN Care Coordinator operates within the HTC's Clinical unit, delivering direct patient care under the guidance of medical providers. This role involves assessing and treating patients with acute bleeding or clotting issues in both outpatient and inpatient settings, and conducting nursing evaluations during annual comprehensive and follow-up clinics. Working as the primary care nurse for an assigned patient caseload, the RN Coordinator implements treatment plans, educates patients and community agencies, participates in outreach clinics and programs, and contributes to clinical research studies. The position requires a commitment to continuous learning in the specialty area of coagulation disorders and the broader fields of nursing and medicine. The HTC is an internationally recognized specialized disease management center for children and adults within the University of Colorado School of Medicine, serving over 3000 patients with rare chronic bleeding and clotting disorders.

Requirements

  • Bachelor’s degree in nursing from an accredited institution.
  • Colorado RN licensure or ability to obtain Colorado RN licensure.
  • Two (2) years of experience as a registered nurse (for Senior Level).
  • Five (5) years of experience as a registered nurse (for Principal Level).
  • Basic life support certification or ability to obtain basic life support certification.
  • Successfully pass a drug test through Children’s Hospital Colorado.
  • Willing and able to pass a national criminal background check.
  • Willing and able to travel to outreach clinics, weekend educational programs, and offsite meetings and trainings.
  • Willing and able to perform some evening and weekend home care or hospital visits.
  • Ability to transport patients as necessary, which may involve lifting 50 lbs. or more.
  • Ability to communicate effectively, both in writing and orally.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.
  • Outstanding customer service skills.
  • Proficiency with Microsoft Office and electronic medical records and databases.
  • Ability to comply with HIPPA regulations.
  • Ability to assist a diverse population of all ages.
  • Ability to train and educate others and work as a team player.
  • Demonstrated interest in serving patients with chronic conditions.

Nice To Haves

  • Ten (10) or more years of experience as a registered nurse.
  • Pediatric advanced life support and/or ACLS certification.
  • Five (5) years of experience in clinical service.
  • Three (3) years of care coordination experience.
  • Hemostasis and thrombosis experience.
  • Advanced IV access and phlebotomy experience.

Responsibilities

  • Make complex daily clinical decisions using physical assessment and laboratory assessment.
  • Determine payment sources for treatment and make follow-up plans for patients managed by physicians, physician assistants, or nurse practitioners.
  • Provide research support to CRA, including blood draws, medical history, and physical exams.
  • Fully participate in and attend outreach clinics, educational programs, and camp, which may involve out-of-town travel, overnight, and weekend time.
  • Provide direct patient care for persons with coagulation disorders under the auspices of physician management.
  • Assess and treat patients with acute bleeding or clotting problems under direction of medical providers, both outpatient and inpatient.
  • Conduct nursing evaluation of patients in annual comprehensive clinics and during follow-up clinics.
  • Function as the primary care nurse for an assigned number of patients.
  • Implement treatment plans.
  • Provide education to patients and community agencies.
  • Participate in outreach clinics and programs.
  • Contribute to clinical research studies.
  • Commit to ongoing learning and education in this area of specialty and the field of nursing and medicine.
