This position is responsible for planning, directing, providing and documenting nursing care to patients of the Children’s Clinics. The role involves adhering to approved standards of triage and nursing practice, facilitating patient appointment scheduling, assessing the severity and urgency of telephone calls, and developing a plan using the triage process and nursing assessment. The Care Coordinator reviews medical records, coordinates the acquisition of patient records and reports, assesses and documents patient intake information, and determines necessary nursing actions. They also provide supervision and guidance to Medical Assistants to ensure effective patient flow, and collaborate with physicians and other care team members in patient assessment and care delivery. Additionally, the role includes participating in discharge planning, performing patient/family teaching, and providing medically necessary patient information to agencies/organizations.
