About The Position

Partnership Health Center in Somerville, NJ is seeking a full-time RN Care Coordinator to work on-site at our Somerset Partnership Health Center. The position is Monday-Friday and pays competitively. The Integrity Health Care Coordinator works in collaboration and continuous partnership with chronically ill or “high-risk” patients, their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. A rewarding and challenging position, the successful candidate will meet the following criteria: The Care Coordinator will be expected to engage with between 10-15 patients per workday with engagement defined broadly as: Face to Face visit Telephonic engagement Engagement with Treating Providers Documentation Research, Analysis, and Care Planning A typical day for the Care Coordinator will entail spending half of the day conducting extended patient meetings or other engagements as described above (approximately 30-45 minutes long). The other half of the day will be spent on follow-up with patients, family/caregiver(s), providers, and community resources via secure email, phone calls, text messages, and other communications.

  • Confidence to communicate and outreach to other community health care organizations and personnel
  • Proficient computer skills, including Microsoft Office (specifically Word, Excel, PowerPoint, and Access)

Responsibilities
