About The Position

As part of the Care Coordination Team, the RN Care Coordinator is responsible for Chronic Care Management of high risk and complex care patients. Emphasis on building relationships with provider, patient and family/caregiver to increase self-management of complex disease states including heart failure/heart disease, asthma, COPD, diabetes and supporting workflows to close gaps in patients' chronic and preventative health care needs. Collaboration with PCP and Care Teams to establish patient-centered goals and a plan of care to support achievement of those goals, closing gaps in care and to meet patient needs during vulnerable times of transition across care continuum or adjustment to new diagnoses.

Requirements

  • A minimum of 4 years of experience as a Registered Nurse is required.
  • Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required.
  • Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
  • Keyboard skills and experience working with EMR and Microsoft Office required.

Nice To Haves

  • Experience in Medical Surgical or primary care nursing is preferred.
  • Experience in critical care, home health or cardiac/pulmonary nursing is preferred.
  • Previous experience as a case manager is preferred.
  • Preference for experience with stroke care/following stroke patients, or experience managing high-risk populations and complex patients.
  • Bachelor's degree in Nursing is preferred.
