About The Position

Guthrie is seeking a Full-Time Care Coordinator (RN) to join our Endocrinology team in Sayre, PA—a busy, specialty-driven practice where you’ll play a key role in guiding patients through diabetes management, thyroid care, and other endocrine conditions. The Care Coordinator RN serves as a clinical team lead and central point of contact for patient care within the Endocrinology department. This is a day-shift, clinic-based position offering predictable hours, no bedside demands, and the ability to build meaningful, long-term patient relationships while collaborating closely with a highly specialized care team. In this role, you will coordinate care for patients with chronic endocrine conditions while leading clinical workflows, triaging patient needs, providing education to patients and families, and collaborating closely with providers and leadership to ensure high-quality, efficient care.

Requirements

  • RN with current license in State where employment is located.
  • Maintains current CPR and other certificates as required.
  • Knowledge of procedures and techniques involved in administering simple medical treatments and related patient care.

Nice To Haves

  • At least 5 years RN experience.
  • 3 years specific to Endocrinology.
  • Leadership experience also preferred with direct responsibility to others.

Responsibilities

  • Serves as a clinical team lead and central point of contact for patient care within the Endocrinology department.
  • Coordinates care for patients with chronic endocrine conditions.
  • Leads clinical workflows.
  • Triage patient needs.
  • Provides education to patients and families.
  • Collaborates closely with providers and leadership to ensure high-quality, efficient care.
  • Monitors and implements nursing workflows.
  • Triages emergency calls.
  • Collaborates with the department director daily on establishing and meeting performance metrics.
  • Directs patient to exam room and prepares patient for exam or treatment.
  • Obtain and record vital signs and other appropriate information such as height, weight, drug allergies, current medications and presenting problem.
  • Reconcile medication list and document on chart as requested.
  • Screen, communicate and respond to patient needs.
  • Triage for emergency calls using approved protocols, ensuring documentation of the phone call meets standards.
  • Anticipate needs of the patient and practitioner during exam.
  • Attends physician during exam and treatment when requested as appropriate for RN licensure.
  • Administers prescribed injections/medications and records them on patient’s chart.
  • Administers IV’s for all ages of patient clientele when requested.
  • Manage patient flow to ensure adequate time for patient care and family education.
  • Routes patient after exam to proper area for ancillary testing or follow‐up appointments, ie. blood work, x‐rays, and enter lab testing as requested.
  • Answers telephone and performs other clerical functions as necessary to include monitoring blood sugars that are called in.
  • Appropriately route to provider when needed.
  • Attends to exam rooms, including assurance that proper supplies are on hand.
  • Ordering of supplies.
  • Monitoring and management of workflows to ensure all medication refills are completed appropriately.
  • Write or call RX’s after obtaining provider authorization for all department providers as needed.
  • Document as written in patient’s chart.
  • E‐mail, Fax or call in any insurance referrals.
  • Follow up phone call with referral number to specialist if requested.
  • Provide education according to evidence‐based guidelines, interactively and collaboratively with the patient, family and care team.
  • Assists in smooth departmental operation by filling in for other LPN’s, clinical assistants, MOA’s and RN’s as requested.
  • Provide follow up information for the patient indicated to ensure compliance with recommendations, medication, lab/ xray, etc.
  • Develop care initiatives to asses and coordinate diabetes care management including counseling, assistance with monitoring chronic and serious medical conditions.
  • Partner with practice director ensuring all patient needs are met, performance standards are maintained, and appropriate staffing levels assigned per day.
  • Must train and maintain proficiency in the use of computer equipment and other computer program training as requested.
  • Other duties as assigned as appropriate for RN licensure.
