About The Position

Opportunity for an RN to join our Women's Health Clinic team as a full-time Care Coordinator. As the Care Coordinator you will work with the team to support patients with chronic conditions and/or complex needs. You will develop, manage, and update highly individualized care plans for individuals at-risk for poor outcomes and more! Nurses are the backbone of Valley View, combining compassionate care with clinical expertise.

Requirements

Current Colorado RN license or ability to obtain.
BLS Certification through the American Heart Association
Effective communication – both verbal and written
Excellent interpersonal skills
Ability to affect change.
Ability to perform critical analysis.
Self-Directed
Positive Attitude
Minimum 2 years RN work experience.

Nice To Haves

Three to four years RN experience in the area of specialty is preferred.
Experience in Case Management a plus.
