The RN Care Coordinator - Intake (PFK) position serves as the primary clinical resource for the Intake team, using assessment and critical thinking skills to review referrals and support clinical decision-making. Approximately 20% of the role supports the Transitions of Care team through outreach calls to patients and families. This role ensures the provision of quality patient care in the appropriate setting through care coordination, case management, utilization management of inpatient admissions, and transitions of care to different levels of care. The RN Care Coordinator collaborates with the attending practitioner, the family, and other members of the health care team to coordinate individualized plans of care and communicates the plan to appropriate stakeholders. The goal is to ensure high quality care is provided as efficiently and cost effectively as possible, facilitating continuity of care by coordinating transition to post-discharge care. The role practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses and maintains a current Ohio Nursing License.
