About The Position

Join St. Luke’s Hospice Team: Where Compassion Meets Care. At St. Luke’s, we are committed to fostering a positive, inclusive, and supportive culture that empowers our Registered Nurses (RNs) to provide exceptional care. As a Hospice Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.

Requirements

ASN or BSN in Nursing
2 years of RN experience
Current RN license in the state of Idaho
Basic Life Support (BLS) certification
Valid driver’s license and proof of auto insurance

Nice To Haves

Hospice Case Management experience
Interest in training for an occasional pediatric patient

Responsibilities

Collaborate with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload.
Deliver in-home care from admission to discharge.
