About The Position

Shenandoah Community Health (SCH) is a Federally Qualified Health Center dedicated to improving healthcare access by addressing challenges such as cost, transportation, and language barriers. We offer a wide array of services including primary care for all ages, women's health, behavioral health, and oral health, complemented by on-site pharmacy, labs, and radiology. It also involves the development and revision of forms to ensure compliance with regulations and standards, as well as meeting team-identified needs. The position will also provide direct patient services and act as a referral source for pharmacy assistance programs.

Requirements

  • Must be a graduate of an accredited College of Nursing School.
  • Must have a current nursing license in each state of desired practice.
  • Annual basic BLS/CPR certification is also required.
  • Must have computer experience in Power Point, Microsoft Excel, and Microsoft Word.
  • Must compile reports, data, and minutes for Peer Review, Performance Improvement, and Patient Satisfaction.
  • Must have excellent skills in patient communication and education.

Nice To Haves

  • BILINGUAL English-to-Spanish and Spanish-to-English A PLUS!
  • Case Management experience preferred.
  • Previous experience working as an RN is preferred.

Responsibilities

  • Coordinating peer review, performance improvement, and patient satisfaction activities.
  • Developing new and revising existing forms as needed to comply with regulations and standards or to meet needs identified by the team.
  • Providing services to patients and acting as a linkage/referral source for applicable pharmacy assistance programs.
  • Demonstrating knowledge and skills necessary to provide ongoing written support for all aspects of medical records; thoroughly documenting Case Management activities.
  • Collecting vital signs and entering data into the NextGen HER as needed.
  • Screening medical requests/patient appointments.
  • Assisting patients by linking and referring appropriately to services not provided by BHS per provider instructions.
  • Maintaining the medication room following BHS policy and procedures.
  • Cross covering group therapy activities as needed.
  • Assisting patients in applying to pharmaceutical companies’ patient assistant programs as appropriate by providers.
  • Acting as liaison to the pharmaceutical company representatives requesting samples and other materials as needed.
  • Logging in samples and PAP medications into the appropriate log per procedure.
  • Periodically checking expiration dates on samples and disposing of expired samples per procedure.
  • Coordinating lunch presentations weekly with pharmaceutical representatives.
  • Maintaining patient confidentiality and privacy according to SCH policies and procedures; using discretion when discussing patient matters.
  • Demonstrating courteous and cooperative behavior.
  • Maintaining positive working relationships.
  • Exhibiting professional standards through appearance and demeanor consistent with SCH standards.
  • Demonstrating thorough knowledge of work assignments and performing assignments in a manner to meet quality and timeliness objectives.
  • Demonstrating knowledge and understanding of SCH policies and procedures.
  • Complying with applicable safety policies and procedures.
