About The Position

This is a part-time Registered Nurse position in Behavioral Health. The role involves providing nursing care to patients, updating treatment plans, and managing chronic health conditions that affect behavioral health. The position emphasizes patient-centered care, team collaboration, and timely documentation. The nurse will also be responsible for medication adherence education, alerting prescribers to patient status changes, performing general nursing duties, and ensuring compliance with all relevant confidentiality laws (HIPAA and 42 CFR, Part 2). The role requires proficiency in EMR and video-conferencing software, and daily use of work email.

Requirements

  • RN with at least 3 years of experience in healthcare setting.
  • Knowledge of chronic physical conditions.
  • Holds valid Ohio professional license as appropriate.
  • Ability to be mobile.
  • Possess a valid State of Ohio motor vehicle operator’s license and be insurable under Firelands’ auto policy.
  • Employee must provide proof of personal automobile liability insurance coverage upon request.
  • Successful completion of a 90-day probationary period.

Nice To Haves

  • Behavioral health experience strongly preferred.
  • Experience working with individuals who have a mental, physical or substance use disorders is preferred.

Responsibilities

  • Responsible for the nursing care of persons on his/her caseload.
  • Updating the Individualized Treatment Plan (ITP) when the patient is receiving medication services only.
  • Provide care management for patients whose chronic health conditions affect behavioral health conditions.
  • Provide patient-centered care in a manner that conveys acceptance, instills hope, and reduces stigma.
  • Positively contribute to patient, referral source and employee satisfaction.
  • Be responsive to patients’ needs.
  • Respond to requests for information and return phone calls in a timely manner.
  • Be a team player who is a positive influence on morale and culture of the workplace.
  • Perform tasks necessary to support revenue including timely and accurate documentation within departmental guidelines.
  • Provide care management such as obtaining information from other healthcare providers as needed, conducting medication reconciliation on a routine basis to ensure the record reflects all medications prescribed for/used by the patient, providing education regarding physical and behavioral health conditions when indicated and within the nurse’s scope of practice; and referring patients to additional behavioral health services when medication services alone are not sufficient for stabilizing symptoms.
  • Provide education and interventions to support medication adherence.
  • Alert the prescriber to concerns regarding changes in physical health status, medications, behavioral health symptoms and/or medication side effects, and lab results indicating critical values.
  • Perform general nursing duties including taking vitals, drawing blood, collecting urine samples, etc.
  • Ensure all medications are stored, tracked and disposed of per departmental policy.
  • Ensure timely completion of reports due to the Pharmacy Department.
  • Understand the role of Patient Care Standards (PCS) as a measure of our commitment to serving those in need and ensuring that access to care is provided.
  • Ensure PCS standards are met and actively utilize and respond to strategies provided through supervision when there are challenges in meeting these standards.
  • Effectively and efficiently utilize EMR and video-conferencing software and any other technology as required for patient care.
  • Utilize work email on a daily basis keeping current with FRMC and departmental communications.
  • Ensure all electronic forms of communication, that include PHI, are compliant with federal laws regarding confidentiality: HIPAA and 42 CFR, Part 2 laws regarding confidentiality.
  • Adapt nursing services to ensure the same standards of patient care are provided when tele-services are provided.
