About The Position

Responsible for assisting the manager in overseeing the resources, operations, and staff associated with hospice patient care to ensure the delivery of high-quality patient care. Provides support to families and patients to develop optimal care plans. Assists in clinical and quality improvement initiatives for hospice services in compliance with regulatory requirements.

Requirements

  • Maintaining licenses and certifications

  • Monitoring and evaluating the performance of hospice staff

Nice To Haves

  • More than 40 years’ experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties.
  • Experience as one of the first hospices in New Jersey and the United States.

Responsibilities

  • Assists the Hospice Director in implementation of the plan of care and oversee the care provided by nursing staff and volunteers.
  • Responsible for assisting in promoting a culture of excellence in patient care.
  • Ensures outcomes are met as related to patient/family/caregiver needs as well as employee performance and engagement.
  • Maintains licenses and certifications while assisting the facility in adherence to federal, state, and professional standards and regulations.
  • Monitors and evaluates the performance of hospice staff.
  • Performs other related duties as assigned.
